Step 4.
Fill out the form completely and correctly. Check the spellings, and other doubtful entries.
Step 5.
Select an option if you have other legal proceedings to your birth certificate. Select honestly.
Step 6.
Fill out the form of the receiver, but first, decide who would receive of the certificate upon delivery. Then select the relationship of the receiver to the certificate that you are applying. I recommend putting yourself for no further future errors.
Step 7.
You'll see your order summary. Check if everything is right.

Step 8.

Step 10.
Be reminded! If you or the receiver you entered on the form is not around during the time that the certificate arrives, you may use the Letter of Authorization provided. Enter the name of your authorized representative and select a valid I.D. of that representative to be presented upon the delivery.
